1. Adopter agrees to pay RR an adoption fee of (please enter adoption fee below) by Cash or Credit Card (please enter payment method below) for the above named dog. This fee includes: Spay/Neuter, microchip, and vaccinations for distemper, parvo and rabies. Adopter agrees to keep all vet appointments. If adopter does not keep appointment for either vaccinations or spay and neuter, OR elects to have vet services provided by a different vet, these REQUIRED vet services will be incurred at adopter’s expense. Due to the nature of rescue, RR has the right to take possession of dog if spay/neuter is not performed by a licensed vet within the time frame agreed upon at adoption. *
2. All dogs under eight years of age have the option to complete a recognized training course to be completed within 3 months of adoption at the adopter’s expense. Once completed, the adopter will receive a $50.00 refund if the training certificate is submitted to training@rescuerovers.org. Adopter agrees to enroll dog in obedience training if needed to assist with successful integration with other animals, people and property. If training classes are deemed a required term of adoption, adopter will schedule and attend classes held by a professional trainer and provide proof to RR upon completion of the class.
3. Adopter agrees the dog is to be a family companion and agrees to provide the dog with a safe INDOOR home, adequate food, water, proper care, exercise, love and attention. Adopter agrees the dog will not be kept as an outdoor dog. Adopter agrees to have a clean, secure fenced area for the dog. If a fenced enclosure is not available, adopter agrees to maintain control of the dog by using a leash. Adopter agrees not to violate any laws or ordinances with the dog. The dog is not to be used as an attack or protection trained dog and will not be used for commercial guard work.
4. The dog is not to ride loose in the bed of pickup trucks or to be left in a car when air temperatures exceed 70 degrees.
5. If the adopter is to go away for a prolonged period of time or vacation, adopter is to prearrange proper care for the dog.
6. Adopter agrees to have the dog examined by a licensed veterinarian at least once each year and provide for all vaccinations and treatments necessary to ensure good health. If euthanasia becomes necessary, adopter agrees to have it performed by a licensed veterinarian.
7. All dogs adopted from RR are microchipped, adopter agrees to register this chip within 7 days of adopting. Adopter agrees to update the microchip with their new contact information within 72 hours of any change in home address or phone. If at any time the dog is lost or stolen, please notify RR by phone or email immediately.
8. RR makes no guarantees or statements regarding the dog's age, breed, health, or temperament. While RR has made every effort to provide accurate history and assessment of the dog, RR is not able to guarantee the dog’s age, breed, medical status, behavior or disposition. RR is available for consultation, advice and assistance pertaining to the health, training and compatibility of the dog. Adopter agrees to release RR and its members or representatives of any and all possible claims arising from injury or damage caused by the dog to any person or property. Adopter accepts this dog as is with all defects, either observable or unobservable, and assumes all risk for the dog upon signing of this contract.
9. Adoptions 100 miles or more from SLC are subject to the following OUT OF AREA policy. Adopter agrees to the terms provided in the Out of Area Addendum to be completed at the time of adoption. Out of Area is defined as any location 100 miles or more from SLC, whether at the time of adoption or due to a move after adoption. If Adopter elects to terminate adoption for any reason you will be required to surrender the dog back to an authorized representative of RR. Any transportation costs incurred will be at the adopters expense.
10. If for whatever reason the adopter is unable to keep this dog for its entire life, the adopter must return the dog to RR subject to availability of a foster. Adopter agrees not to give away, sell or dispose of the dog without express consent of RR. If the dog is returned directly to an authorized agent of RR, in the same health condition and within 14 days of the adoption, RR will issue a refund of $200.00 of the adoption fee. After 14 days, no refund of any amount will be issued. No refunds for Senior or other reduced fees. If thru the return process the dog is deemed unsafe to humans and/or other dogs, euthanasia may be recommended which will be at the adopter’s expense.
11. Adopter will allow RR to conduct home visit if requested in order to approve adoption application. Adopter will allow phone check-ins after adoption, and consent to a scheduled home visit if required in order to check on the dog’s welfare after adoption.
12. This agreement shall inure to the benefit of and be binding upon adopter and RR and their successors or beneficiaries. Adopter has fully read and understands this contract and all of the agreements, which sets forth all promises, conditions, and understandings, oral or written. In the event adopter breaches this contract, including all its covenants and conditions, RR has the right to rescind this contract, demand the return of the dog, keep all adoption fees, and be reimbursed for expenses associated with safely returning the animal to RR.
