This agreement is entered into this date by and between Rescue Rovers (“RR”) and the applicant (“Adopter”) for the adoption of the above identified rescue dog (“the dog”). Spay/neuter, DHPPV, Bordetella and Rabies vaccines, along with Microchip are required as per our standard adoption contract.

This will require that the dog not be left unsupervised, and most importantly that the dog is not able to run loose or have contact with unspayed females and/or unneutered males until they are fixed.
This agreement shall inure to the benefit of and be binding upon Adopter and RR and their successors or beneficiaries. Adopter has fully read and understands this contract and all of the agreements, which sets forth all promises, conditions, and understandings, oral or written. In the event Adopter breaches this contract, including all its covenants and conditions, RR has the right to rescind this contract, demand return of the dog, keep all adoption fees, and be reimbursed for expenses associated with safely returning the animal to RR.
Spay and Neuter of Salt Lake located at 4815 S Center St, Salt Lake City, UT 84107 Drop off time between 7:00 to 8:30 am. Pick up time 4:30 to 5:30 pm. NO food after 10:00 pm the night before surgery.
